What is EERS/EyeV?
The Eye Care Electronic Referral System is a referral, advice and guidance, and image-sharing system. Its connects primary care optometrsists with providers of NHS specialist and hospital eyecare services in Bradford, Airedale and Craven.
Long term this will help achieve significantly improved two-way communication between primary and secondary eyecare providers, and by replacing multi-step manual processes with streamlined, end-to-end referrals. This will bring greater efficiency to the referral and triage process, reducing wait times and improving patient experience and outcomes.
Once you are registered you can send your GOS18 referrals on the EyeV platform by either uploading the GOS18 or completing the relevant information within the portal.
You can continue to upload your current GOS18 or other local referral forms on to EyeV.

PATHWAYS:
Multiple pathways and providers are now live on EyeV. Once made, the patient can either expect to be contacted directly by the provider with an appointment or they will be expected to book their own appointment with the provider after receiving a booking letter by post.
EyeV have added details of which process will occur on the platform. They are also copied below.
Please note that if the patient is referred to a directly bookable service, that department will not have any patient details visible to them until the patient books their appointment.
The patient letters should be received within a few days by post. If the patient hasn’t booked an appointment in two weeks, a second letter will be sent. Copies of the patient letters are now added to the patient EyeV referral and can be printed off by the super admin if required.
The letters state that the patient has been referred by East Midlands Medical Services to your chosen provider. This is our EyeV referral platform. You may wish to inform your patient to look out for this letter in the coming weeks.
We have consulted our representative bodies about where responsibility lies in the referral process. If there is insufficient information for the hospital to process the referral, (e.g., missing patient details), then responsibility lies with the referring practice. If there is evidence of HES review and decision making, (eg rejection following triage) then responsibility lies with the HES. With regard to the “direct to list” appointments where the patient has to book the appointment to proceed, this should be clearly explained to the patient and noted on the record card. Where there is a risk of harm to the patient from delay or failure to book, it is advisable to provide a clear safety netting process. The AOP has a template for this.
Please note that patients are not contacted by the hospital if the referral is rejected. In such cases, we are expected to notify our patients. We have also been advised by our representative bodies that referring practitioners should check EyeV on a regular basis to monitor progress of their referrals. This is a good way to “stay safe”. Referring practitioners are recommended to monitor referral progress at each clinic they work. This recommendation could be communicated through the LOC. Please chase any referrals that have not progressed to triage after 4 weeks on the tracker unless these are awaiting patient action and a patient letter is visible. We have made EyeV aware that progress bar update emails are not being sent, and that it is not always clear if a patient has been “accepted”. We expect to see changes soon.
The progress bar will only change as far as “triaged” for now. Negotiations with secondary care are needed to commission this extra step.
If you are doing a direct cataract referral, please complete the Pre-op enhanced optical services EOS form rather than the GOS18. We have asked for some updates to this form which we hope to see soon.

A The hospitals will triage routine referrals within 1-2 weeks. You should raise a ticket on EyeV if the status has not changed after 4weeks. The referral confirmation letter tells patients to call the hospital if they have not heard within 40 days. Wet ARMD will be triaged within a week and the patient contacted. Depending on referral type/location the patient may receive an appointment or be required to book their own. See the EyeV platform or our website for details. Please inform patients that letters say they have been referred by East Midlands Medical Services EMMS (this is EyeV)
